Required Vaccines and Exams

Required exam information for students in various grades is provided in the table below.

Dental Exam

Anytime during the Kindergarten and 3rd grade school year.

· Only dental exams dated on or after August 1st of the previous school year of the required exam.

Physical Exam

Anytime during the

Kindergarten school year.

· Only physical exams dated on or after August 1st of the previous school year of the required exam.

In School Medications

If your child needs to take medication in school, whether it is an Epi-Pen, inhaler, daily medication, or as needed medication, the parent and physician must complete the Permission to Administer Medication form and the medication must be provided to the health office staff in the original bottle labeled from the pharmacy. It is helpful to have all medications and Permission to Administer Medication forms dropped off at school before the first day.

Finally, although physical and dentals forms are not required for the first day of school, it is best to get them completed and turned into the nurse’s office as soon as possible. You will receive notices that these exams are due until they are completed.
